Parker Point Medical Center helps patients complete the required medical review, vaccination assessment, testing, and documentation based on current U.S. Citizenship and Immigration Services instructions.
Because requirements and forms can change, contact the clinic before your appointment to confirm what to bring, which services are included, and whether any testing or vaccinations may involve additional fees.
What to bring
Government-issued identification

Vaccination records and translated copies when applicable
Relevant medical records and a current medication list

After the examination
The clinic will complete the required medical documentation after all necessary examination components, testing, and vaccination requirements are addressed. Do not open a sealed envelope if USCIS instructions require it to remain sealed. The immigration medical exam is not a substitute for comprehensive primary care.
